February 11, 2020 Case Management and Care Coordination Webinar Part 1 from 12:00 p.m. - 1:30 p.m. Central

Purpose: Under PDGM, the needs of the patient will require prioritization among disciplines – therapy will still play a key role in patient safety and improving functional abilities.  The main component of payment will be the primary diagnosis: choose wisely, determine secondary comorbidities, and know how to validate all diagnoses listed on the plan of care. Setting goals that are reasonable and achievable, addressing the concerns of the patient and family, and developing a plan of care to utilize all disciplines to reach those goals will be the key to success in PDGM.

Learning Objectives:

  1. Identify patient needs based on assessments by all disciplines.
  2. State one factor in choosing diagnoses for the Plan of Care.
  3. Develop a plan with integrated interventions to achieve goals.

February 18, 2020 Case Management and Care Coordination Webinar Part 2 from 12:00 p.m. - 1:30 p.m. Central

Purpose: PDGM will demand effective case management that reduces duplication and waste, maximizes utilization of all disciplines in a collaborative way, ensures the patient’s needs are met and goals reached, and keeps your agency viable in a challenging environment. Case management by field staff patient care managers can lead to out of control visit frequencies and missed visits, lack of progress toward goals, and poor performance on outcomes. Examine the centralized approach to case management that uses standardized processes and tracking of key monitors to avoid surprises under PDGM.

Learning Objectives:

1. State one principle of case management.
2. Discuss pros and cons of centralized case management model.
3. Identify key information to evaluate effectiveness of case management.
